We are a team of dedicated, friendly, and professional carers who have the needs of those we support at the forefront of our work. Everything we do for individuals is tailored to support their complex and varying needs.
We like to think of ourselves as an extended family. We look after those we support with the utmost care and professionalism with regards to their medical, physical, cultural and emotional needs, and we also provide a home-from-home environment in which they can welcome their family and friends, who are as much part of the organisation as the staff themselves.
Our History
Horizon Healthcare Homes Ltd was started in 2006 by husband and wife team, Roger and Shona Williams.
Their aim was simple, and yet complex, a desire to build and run the best possible residential care facilities for adults with both learning difficulties and physical disabilities across the North of England.
The first home, Langley Lodge, opened in 2007. It was followed by Hampton House in 2009, Cranmer Court in 2010, Ferndale Lodge in 2011, Beckside Lodge in 2013, Windsor Court 2017 and Fixby Lodge 2017.
Today there are nine Horizon Healthcare Homes in Leeds, Bradford, Dewsbury and Huddersfield following the opening of our two new sites Roxburgh Lodge in February 2021 and Sandhurst Court in April 2021.
Thanks to the complete obsession of the founders with attention to detail, and with having the most cutting edge services and facilities, coupled with a unique home-from-home atmosphere and environment, Horizon Healthcare Homes are now at the forefront of this industry within the area, and plan to continue organic development and growth in response to the demand for high quality specialist residential services for people with Learning Disabilities.

Aims and Objectives
Horizon Healthcare Homes is a privately owned and operated provider of residential support services for people with learning disabilities. There are currently nine CQC registered services in the West Yorkshire region; five of these are located in Kirklees, three in Bradford and one in the Leeds area. The most recent addition to the company in March 2021 is Sandhurst Court and is situated in Dewsbury
The aim of the organisation is to offer care and support to people with learning disabilities and associated complex needs of the highest standard possible and to enable each person to reach their full potential and enjoy life.
The mission statement of Horizon Healthcare Homes is: ‘Passionate about achieving high quality care in a homely environment’. This is based directly upon the four core values consistently identified during a survey across the organisation: Passionate, Achieving, Caring, and Homely.
Each home has a real focus upon implementing person centred approaches and involving people as much as possible to offer choice and control regarding what happens in their lives. Where people do not have the capacity to make decisions then there are robust procedures in place to ensure that Best Interests are clearly identified and recorded. This will include involving any contributions that the person is able to make as well as those people considered to be a part of that persons ‘circle of support’.
Care and support will:
- aim to meet the identified needs of each individual supported
- promote and encourage independence of those supported and enhance individual rights, choice and control
- offer privacy, respect and dignity to each person supported
- recognise and embrace diversity within people’s individual ethnic, religious, social and personal preferences
Each service is purpose built and offers support 24 hours per day, 365 days per year. The size of the accommodation ranges from 6 beds to 12 beds. Each service has adaptations to accommodate people with physical disabilities and is designed and furnished to the highest standard, providing a very pleasant, homely environment for people to flourish within. All bedrooms have en-suite facilities. The buildings are of very generous proportions, which enable people to obtain space when they prefer this. There are shared kitchen and living room spaces, as well as a separate ‘sensory bathroom’ and additional sensory / relaxation room in each service. Ceiling tracking is available in bedrooms as well as profiling beds for those individuals that require these facilities. Each home also provides generous outside space and a vehicle which is designed to accommodate wheelchair access to enable all individuals to engage in community-based activities.
Any prospective transition into a service will be proceeded by a detailed assessment of the person’s needs and aspirations. An impact assessment will determine the suitability of offering a placement to a person. The decision will be based upon how effective the service will be able to support that individual, the impact upon others, including other people supported, as well as the staff team. If it is decided that a placement would be appropriate, then a detailed transition would be planned and a Person Centred Plan formulated based upon the information gathered throughout the assessment process. This plan is then reviewed periodically via person centred planning processes to ensure that the plan remains valid and current. The individual person that we support is included as much as possible within these processes.
Family members and friends are welcome to visit any of the services at any reasonable time, with consideration to the needs and wishes of those people living within the services. We acknowledge that the people we support, family members, friends, advocates and other involved people may want to comment on the service we provide and this is actively encouraged. There are therefore clear, complaints, compliments and comments procedures in place which are accessible, as well as periodic questionnaires that are sent out as a part of the company’s quality assurance system.
All staff will have a satisfactory DBS in place as a part of the robust recruitment and selection process. All staff will complete and hold the Care Certificate as well as undertake a comprehensive induction and training program. All staff are subject to a 12-month probationary period in which a person’s suitability to the role will be assessed. Regular supervision and support, as well as continued professional development will continue throughout a staff members employment.
Each Horizon Healthcare Home will have a suitably qualified and experienced Registered Manager in place who will be supported by a Deputy Manager and an appropriate number of Team Leaders. The management team will be responsible for ensuring that the people supported within the service receive the required levels of support to ensure their ongoing safety while allowing appropriate levels of risk to be taken, continued development and enjoyment. An on-call rota is in place to ensure that each service has adequate management support at all times.
The Director of Operations, Managing Director, and recently appointed Operations Manager, will provide additional and ongoing support to each of the services and will have overall responsibility of ensuring that each home carries out the functions as described within this statement of purpose as well as continuing to develop and strengthen the company as a whole. The Senior Management Team will be actively involved in the day to day operations of the business and will have a regular presence in each of the services to ensure that the homes are governed appropriately. Quality Assurance audits will be undertaken periodically by the Operations Manager to monitor the performance of each home against regulatory standards.
